Abstract
Haitian and Haitian American adults with advanced chronic illnesses in Massachusetts may face barriers to palliative care, including limited awareness, language differences, low health literacy, cultural beliefs, and misconceptions about its purpose. These barriers often delay discussions about symptom management, advance care planning, and supportive services, limiting opportunities to improve quality of life earlier in the disease course. Persistent racial and ethnic disparities in serious illness care further highlight the need for more equitable approaches to palliative care education and referral. This policy brief proposes that Massachusetts community health centers implement a standardized policy providing culturally appropriate Haitian Creole educational materials, professional interpreter services, and teach-back during routine primary care visits. The proposed policy aims to improve understanding of palliative care, reduce misconceptions, encourage earlier referrals, strengthen patient and family engagement, and advance health equity for Haitian and Haitian American adults with serious chronic illness.
